POSITION SUMMARY: Independent Medical Group, LLC (IMG) specializes in the testing, treatment, and management of HIV, Hepatitis C, and Sexually Transmitted Diseases. Co-founders Clifford W. Knights and Steve Vixamar have built a partnership with the Centers for Disease Control and Prevention (CDC) to level the playing field for all high-risk Americans, no matter their race, sexual orientation, or socio-economic status. IMG offers an array of services that includes healthcare and pharmacy services, case management, and mental health and substance abuse management. The Nurse Practitioner is responsible for providing care to clients by assessing clients' health status, conducting physical examinations and evaluations, providing treatment, and prescribing medications, recommending follow-up care, and making referrals to appropriate health professionals. The employee will serve as a clinical authority for Independent Medical Group clinics. In addition, the employee will provide health teaching, patient education, counseling, and guidance. The incumbent in this position demonstrates considerable discretion. The Nurse Practitioner works under direct supervision of the Nurse Practitioner Manager who provides administrative supervision, and under the technical direction of a physician.

ESSENTIAL DUTIES & RESPONSIBILITIES:

Performs physical examinations and preventive health measures within prescribed guidelines and instruction. Working knowledge of HIV and HEP C, and STI transmission and treatment.

Occasionally function in a mobile clinic setting including diverse locations and possibly including non-traditional clinic hours.

Works closely with the practice manager and case manager in scheduling appointments for telehealth.

Performs patient evaluation, diagnosis, initiation of PrEP for HIV prevention, and treatment of STIs utilizing the Electronic Medical Record (EMR).

Works in collaboration with a physician in formulating treatment plans for health problems, including referrals and/or follow-up.

Develops, implements, and evaluates a plan of care that is appropriate for the patient in collaboration with patient case manager.

Assisting physicians with the performing history and physical examinations including, but not limited to, dermatologic, head, ear, nose throat assessments for infectious disease.

Completes patient follow up visits to include detailed, timely documentation including patients' progress with medication and/or treatment.

Works in collaboration with a physician in formulating treatment plans for health problems, including referrals and/or follow-up. Writes prescriptions for medication based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols.

Refers or seeks consultation with physician when a patient has a condition for which the nurse practitioner is not credentialed.

Assist with conducting routine and ad hoc QI/QA assessment and Peer Review.

Works in collaboration with the management team to address clinical operational issues consistency across IMG practices.

Participates in provider meetings and staff meetings as required.

Requires a professional and interpersonal skill to interact with clients, and healthcare professional

Requires knowledge of varied social, economic, cultural, and lifestyle characteristics and the ability to communicate with clients as well as target groups of diverse individuals.

COMPUTER EQUIPMENT & SOFTWARE SKILL REQUIREMENTS:

Full knowledge of Windows Office Suite; particularly Microsoft Word and Excel

Full knowledge of EMR/EHR systems and telehealt h .

MINIMUM REQUIRED QUALIFICATIONS & EXPERIENCE:

Education: Masters Prepared and Certified Nurse Practitioner- Registration and Licensure by the State of Florida Board of Registered Nursing is required.

Solid knowledge of EMR/EHR systems and telehealth.

Perform Specimen collection/ Blood and Urine for STD/HIV Testing

Strong knowledge of PrEP (Pre-exposure prophylaxis)

Excellent communication and interpersonal skills

Highly organized and a proven strategic thinker with ability to engage and motivate teams

PREFERRED QUALIFICATIONS & EXPERIENCE:

Education: Masters Prepared and Certified Nurse Practitioner- Registration and Licensure by the State of Florida Board of Registered Nursing is Required.

Experience: Two or more (2+) years of professional work experience as a Nurse Practitioner is preferred. Experience in the care and treatment of HIV and HEP C, and STI transmission in a primary healthcare setting is highly preferred.

Certification(s): Relevant certification (e.g. National Phlebotomy Certification) is preferred

§ Proven professional work experience as a Nurse Practitioner involved in the care and treatment of HIV, HEP C, and STI transmission in the medical field

2 years of experience in primary health care setting

1 year of Disease Intervention or related field and/or equivalent professional experience.

Proof of continued education such as certifications

1 year of extensive working knowledge of STD’s/HIV/HEP C
